Partnership for a Healthy Lincoln, Partners Receive 5 Year, $3.3 CDC Grant

In 2018, the Centers for Disease Control and Prevention awarded Partnership for a Healthy Lincoln a five-year, $3.3 million REACH grant, one of only 31 awarded nationally. This award will help support coordinated health improvement projects that address significant disparities in health outcomes within Lincoln’s minority communities and low income neighborhoods, through nutrition, physical activity, and aligning healthcare providers and community outreach efforts to connect patients with health resources.
